QUALIFICATIONS:

  1. Must be a Senior

  2. Must have an accumulated G.P.A. of 3.5 or better on a 4 point scale.  (convert if necessary)

  3. Must have completed 6 semesters of work

  4. Coach must be a M.H.S.S.C.A. member prior to the start of district play for the current season.

  5. Individual student nominations may be completed anytime after the end of the sixth semester, but completed forms must be received no later than May 15 for boy's and May 15 for girl's.

  6. This individual academic form must be completed for every individual that is nominated.
