At the end of the sixth semester for seniors or at the end of the first semester in grades 9-11, this form should be completed.  All varsity athletes on the high school team must be listed and included in your team average. A copy of the M.H.S.A.A. eligibility list must be attached to this form.  The minimum G.P.A. is 3.3 for nominations.  This form  must be received no later than May 15.  G.P.A.'s are cumulative.
